HOSPITAL TO HOMELESSNESSWhen THC was discharged from a long hospital stay, he found himself without a home. His tenancy had ended during his admission - through no fault of his own - and he was facing serious physical health challenges, a language barrier, and a sharp decline in mental wellbeing. Fortunately, a referral to College Road ABEN meant he avoided rough sleeping and was given a safe, supported place to recover.

FIRST STEPS AT COLLEGE ROADAt first, THC struggled. He was in constant pain, had limited ability to communicate in English, and found everyday tasks overwhelming. But despite these obstacles, he showed remarkable determination to improve his situation.

FINDING HIS VOICE AND CONFIDENCEWith support from staff, THC began actively engaging - using translation tools, learning to manage his appointments and medication, and showing a genuine desire to build a better life. As trust grew, his confidence followed. THC opened up, expressed clear goals, and worked hard to rebuild both his health and independence.

MOVING TOWARDS INDEPENDENCEStaff helped THC access benefits such as PIP and Limited Capability for Work, supported him through GP and hospital appointments, and assisted with correspondence he couldn't yet manage on his own. Once he was ready, he moved into supported accommodation, where his progress accelerated. He formed new friendships, demonstrated resilience, and began taking more control of his life. He later moved into our step down accommodation, continuing to build on his independence.

THRIVING IN HIS OWN HOMEEventually, with the support of SSG, THC secured his own tenancy in a private rented property. With help from an SSG Resettlement Worker, he set up utilities, learned to manage bills, and developed the skills needed to live independently.
Today, THC manages his home confidently, communicates directly with his landlord, and continues to look after his health - with the knowledge that support is available if he ever needs it again.
